Your full name
The titles of the courses you successfully completed with us
Your phone number (with area code)
Your Care Courses student ID number - if you have one (located in the upper right-hand area of your Care Courses certificates or your Care Courses personal online account)
The address where you want us to send your completed Ohio In-Service Form: Email or Mailing Address (including City, State and Zip Code)
